Pre-arrival registration

Complete your registration, medical profile and liability release before you arrive. Each adult participant completes and signs their own form for their booking.

After your booking is confirmed, a private form link is emailed to the booking contact when email delivery is available. For other adults, contact reservations with your booking reference and each participant’s own email address so the team can issue separate invitations. Please do not share medical answers by ordinary email.

Invitation links expire after 30 days and open once. If a link has expired or you need to correct a signed form, contact the resort for a replacement.

What you will need

Your name, birth date and contact details.
An emergency contact and details of allergies, medical conditions and medications.
Your dive certification and experience, if applicable.
Your own full name to sign the release electronically.

This online form is for adults aged 18 and over. For anyone under 18, contact the resort to arrange parent/guardian paperwork and confirm age, supervision and activity requirements. Do not use the adult form for a child.

WAIVER & LIABILITY RELEASE & ASSUMPTION OF RISK AGREEMENT

I am aware of the risks and hazards associated with any water sports, including scuba diving, snorkelling, kayaking, paddle boarding, fishing and boating activities. By signing this agreement, I release Isla Marisol Resort and related persons from all liabilities whatsoever, for personal injury, property loss or wrongful death. I agree that I am giving up my right to sue the released parties but also any rights my heirs, assigns, or beneficiaries may have to sue the released parties. I am fully aware of this document and releases and sign it being conscious of its importance.
